A New Website for Pediatric Care Coordinators
The Minnesota statewide Pediatric Care Coordination: Community of Practice provides a way for care coordinators of children and youth with special needs to network, collaborate, share, and learn from each other. A new website (https://www.mnpedcares.com) now makes this easier. Those interested in being part of this community can register to be a free member.
This website offers:
Upcoming events and trainings (most are free)
Resources (from materials to a list of community organizations)
A directory, full of helpful information
A forum for members to share ideas
Open funding opportunities through the Grant Alert
Each quarter, the website hosts a free, one-hour online event focused on specific issues. Priscilla Weigel, the Executive Director of the Center for Inclusive Child Care, led a webinar on Navigating Child Care for Children with Special Needs: Strategic Advice in the first quarter. On July 28, Jerry Mellum, Senior Planning Analyst for Hennepin County, spoke about Services and Supports for Children and Youth with Disabilities in Minnesota.
The website and these events can be accessed by those who register, such as:
Care Coordinators, Care Navigators, Health Coaches at Primary Care Clinics: Family Practice, Pediatrics, Med-Peds
Care Coordinators at Specialty Care Clinics
School Nurses, Public Health Nurses
Case Managers: county, non-profit, school, etc.
Social Workers, Waiver Case Managers
Anyone... providing care coordination activities and wanting to network with others!
When you log in after you register, you are added to the website forum where you can collaborate and share information. As care coordinators connect, share, and learn from each other, we can improve positive health outcomes by building the capacity of all systems that serve families of children and youth with special health needs.
