A Free Statewide Forum for Pediatric Care Coordinators

 

Care coordinators of children and youth with special needs throughout the state of Minnesota can now network, share, and learn from each other through a free website forum. Those interested in being part of this community forum can register to be a free member: https://www.mnpedcares.com.

Forum discussions so far have included:

The forum is part of the Minnesota statewide Pediatric Care Coordination: Community of Practice. The website with the forum also includes upcoming events and trainings (most are free), resources, a directory, and open funding opportunities.

The forum and website is for care coordinators, care navigators, health coaches at primary clinics, care coordinators at specialty care clinics, school nurses, public health nurses, case managers (county, nonprofit, school, etc.), social workers, waiver case managers. It’s for anyone providing care coordination activities and wanting to network with others.
